Address Format

Address should always be formatted as below.

Department Name(s) [list smallest to largest] Street Address, Room Number/Building Abbreviation (Mail Code) City, State ZIP

Example of correct address:

  • Always have the room number before the building abbreviation. You may include the word "Room" or "Suite" if needed.

  • In the event you want to place the room/building on its own line for any reason, it should go above the street address, not below. The street address and city/state/zip should never be separated.

  • Example College of Medicine Financial Aid Office Room 163 CME (MC 782) 808 South Wood Street Chicago, IL 60612-7302

Mail Codes

Mail Codes are important part of the mailing address and should be formatted as such (MC 783).
