### Address Format

Address should always be formatted as below.&#x20;

Department Name(s)  \[list smallest to largest]\
Street Address, Room Number/Building Abbreviation (Mail Code)\
City, State ZIP<br>

**Example of correct address:**

College of Medicine Financial Aid Office\
808 South Wood Street, 163 CME (MC 782)\
Chicago, IL 60612-7302\

Chicago, IL 60612-7302

* Always have the room number before the building abbreviation.  You may include the word "Room" or "Suite" if needed.
* In the event you want to place the room/building on its own line for any reason, it should go above the street address, not below. The street address and city/state/zip should never be separated.

  Room 163 CME (MC 782)\
  808 South Wood Street\

### Mail Codes

Mail Codes are important part of the mailing address and should be formatted as such **(MC 783)**.
